Hogyan hat
Bispecific antibody that binds the extracellular domains of HER2 and HER3, inhibiting HER2:HER3 dimerization and preventing NRG1 binding to HER3. It decreases cell proliferation and PI3K-AKT-mTOR signaling, and mediates antibody-dependent cellular cytotoxicity.
Szokásos felnőttadag (alkalmazási előírás szerinti összefoglaló)
NRG1 fusion-positive NSCLC, pancreatic adenocarcinoma or cholangiocarcinoma: 750 mg IV infusion over 4 hours every 2 weeks until disease progression or unacceptable toxicity
Terhesség és szoptatás
- Terhesség
- Unsafe Can cause fetal harm: HER2/HER3 loss causes malformations and embryo death in animals, and HER2 antibodies have caused low amniotic fluid. Avoid; check pregnancy status and use contraception during and for 2 months after the last dose. If exposed, monitor amniotic fluid.
- Szoptatás
- Unsafe No milk data; as an IgG1 antibody little is expected in milk and it is probably digested. US label: weigh benefits, allowing for the 2-month washout. LactMed: manufacturer advises no breastfeeding during treatment and for 2 months after. Do not breastfeed until 2 months after.
Jelentett mellékhatások
- Diarrhea
- Musculoskeletal pain
- Fatigue
- Nausea
- Infusion-related reactions
- Dyspnea
Fekete dobozos figyelmeztetések
- Embryo-fetal toxicity: exposure during pregnancy can cause embryo-fetal harm. Advise patients of this risk and the need for effective contraception.
Kinek kell óvatosnak lennie
- Serious and life-threatening infusion-related reactions, hypersensitivity and anaphylaxis can occur. Give in a setting with emergency resuscitation equipment and monitor during and for at least 1 hour after the first infusion.
- Serious and life-threatening ILD/pneumonitis can occur. Monitor for new or worsening pulmonary symptoms.
- Left ventricular dysfunction can occur. Evaluate LVEF before starting and monitor at regular intervals. It has not been studied in patients with LVEF below 50% or clinically significant cardiac disease.
