Gemfibrozil roughly doubles selexipag exposure and greatly increases exposure to its active metabolite. This raises the risk of prostacyclin-type adverse effects such as headache, hypotension, flushing, nausea and diarrhea.

Gemfibrozil and Selexipag are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Gemfibrozil inhibits CYP2C8, which hydrolyzes/oxidizes selexipag and its active metabolite, reducing their clearance.
