Per cosa si usa
Come agisce
Epoetin theta is identical in amino acid sequence to endogenous erythropoietin, the primary regulator of erythropoiesis. It acts on the erythropoietin receptor on erythroid progenitor cells in the bone marrow as a mitosis-stimulating factor and differentiation hormone.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Anaemia of chronic renal failure, correction phase, subcutaneous: 20 IU/kg 3 times per week; may increase after 4 weeks to 40 IU/kg 3 times per week if Hb rise is inadequate (< 1 g/dl within 4 weeks); further 25% increases monthly; maximum 700 IU/kg per week
Gravidanza e allattamento
- Gravidanza
- Unknown Limited data (under 300 pregnancies); animal studies with other epoetins showed no direct harm. EU label: as a precaution, preferable to avoid in pregnancy.
- Allattamento
- Safe Not known if it passes into milk; in newborns, erythropoietin given with milk is not absorbed. EU label: decide whether to stop breastfeeding or treatment. LactMed (epoetin alfa): no special precautions needed.
Effetti indesiderati segnalati
- Hypertension
- Influenza-like illness
- Headache
- Skin reactions
- Hypertensive crisis
- Shunt thrombosis
Controindicazioni
- Hypersensitivity to the active substance, other epoetins and derivatives, or any excipient
- Uncontrolled hypertension
Chi deve prestare attenzione
- Pure red cell aplasia from neutralising anti-erythropoietin antibodies; these cross-react with all epoetins, so affected patients should not be switched to epoetin theta
- Hypertension and hypertensive crisis; control blood pressure before and during therapy
- Severe cutaneous reactions (SJS/TEN): withdraw immediately; never restart after such a reaction
