Sintesi
NSAIDs can reduce pemetrexed clearance and increase its toxicity, including myelosuppression, renal and gastrointestinal effects. Diflunisal has a relatively long half-life, which prolongs the period of risk, especially in patients with reduced renal function.
Cosa significa questa interazione in parole semplici
Diflunisal and Pemetrexed can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Diflunisal belongs to Musculoskeletal; Pemetrexed belongs to Oncology.
Perché avviene
NSAID-related reduction in renal blood flow and glomerular filtration, and possible competition for tubular secretion, decreases pemetrexed elimination.
Cosa fare
Avoid diflunisal for several days before, on the day of, and after pemetrexed administration, following labeled interruption guidance for long half-life NSAIDs. If it cannot be avoided, monitor blood counts and renal function closely.
