Sintesi
Loop diuretics can cause hypokalemia and hypomagnesemia, which sensitise the myocardium to digoxin and increase the risk of toxicity and ventricular arrhythmias. The interaction is well documented and clinically important, especially in older patients and those with renal impairment. Digoxin concentrations themselves are not usually changed.
Cosa significa questa interazione in parole semplici
Digoxin and Torsemide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Digoxin belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Perché avviene
Pharmacodynamic: diuretic-induced potassium and magnesium depletion increases digoxin binding to Na+/K+-ATPase and enhances its arrhythmogenic effects.
Cosa fare
Monitor serum potassium and magnesium regularly, keeping potassium in the normal to high-normal range. Supplement electrolytes or add a potassium-sparing agent if needed.
