Overview

Severity: Major · Evidence level: High

Loop diuretic–induced hypokalemia and/or hypomagnesemia increases susceptibility to digoxin toxicity and ventricular arrhythmias.

Mechanism of action

Pharmacodynamic: electrolyte depletion enhances digoxin’s arrhythmogenic and toxic effects; pharmacokinetics generally unchanged.

Clinical management

Monitor and maintain K+ and Mg2+ (e.g., K+ >4 mEq/L, Mg2+ >2 mg/dL); consider supplementation or adding a K-sparing agent; check digoxin serum concentration after diuretic changes; monitor ECG and toxicity symptoms.
