Sintesi
Cobicistat is a strong CYP3A4 inhibitor that increases salmeterol exposure, with risk of QT prolongation and arrhythmia. It also raises systemic fluticasone levels, so Cushing's syndrome and adrenal suppression can occur with prolonged use.

Elvitegravir / cobicistat / emtricitabine / tenofovir df and Fluticasone-salmeterol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Strong CYP3A4 inhibition by cobicistat reduces clearance of salmeterol and fluticasone.
