概要
Ketoconazole markedly increases conivaptan exposure and the combination is contraindicated in labeling. Excess conivaptan can cause overly rapid rise in serum sodium, with risk of osmotic demyelination, as well as hypotension and infusion-site reactions. Patients with hyponatremia are most at risk.
この相互作用をわかりやすく説明すると
Conivaptan and Ketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
なぜ起こるのか
Ketoconazole is a strong CYP3A4 inhibitor, and conivaptan is a CYP3A4 substrate, so clearance falls and exposure increases.
対処法
Do not use together. If an azole antifungal is needed, consider one without strong CYP3A4 inhibition, or select alternative hyponatremia therapy.
