Concurrent use can cause profound, potentially life‑threatening hypotension with risk of syncope and myocardial ischemia/infarction.

Isosorbide mononitrate and Sildenafil are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

なぜ起こるのか
Additive vasodilation: organic nitrates increase NO→cGMP; sildenafil inhibits PDE5, preventing cGMP breakdown, leading to marked BP reduction.

Do not coadminister. Avoid nitrates for at least 24 hours after sildenafil.
