Resumo
Tricyclics block norepinephrine reuptake and sensitise adrenergic receptors, so systemic epinephrine can cause exaggerated hypertension, tachycardia and arrhythmias. The effect is most important with parenteral or large doses, whereas small amounts in dental local anaesthetics carry less risk.
O que esta interação significa em palavras simples
Amitriptyline and Epinephrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amitriptyline belongs to Central Nervous System; Epinephrine belongs to Endocrine.
Por que acontece
Tricyclic blockade of catecholamine reuptake potentiates pressor and arrhythmogenic responses to epinephrine.
O que fazer
Use the lowest effective epinephrine dose with continuous blood pressure and ECG monitoring when systemic use is needed. Limit epinephrine in local anaesthetics and avoid intravascular injection.
O que perguntar ao farmacêutico
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
