摘要
Phenytoin is an enzyme inducer that reduces isavuconazole exposure, risking loss of antifungal efficacy. Isavuconazole may also modestly raise phenytoin levels.

Isavuconazole and Phenytoin are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Phenytoin induces CYP3A4/5, increasing isavuconazole metabolism.

Avoid the combination and use a non-inducing anticonvulsant such as levetiracetam, or another antifungal. If it cannot be avoided, monitor isavuconazole and phenytoin concentrations closely.
