Summary
Aspirin inhibits platelet function, and alteplase dissolves fibrin clots, so together they raise the likelihood of serious bleeding. Aspirin is often given alongside thrombolysis in acute coronary syndromes, but the bleeding risk, including intracranial hemorrhage, is increased.
What this interaction means in plain words
Alteplase and Aspirin / citric acid / sodium bicarbonate can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive pharmacodynamic effect: irreversible platelet cyclooxygenase inhibition combined with plasminogen activation and fibrinolysis.
What to do
Use together only where indicated, such as acute MI or stroke protocols, and follow local thrombolysis protocols on antiplatelet timing. Monitor closely for bleeding and neurological change, and avoid other additional hemostasis-impairing drugs.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
