Summary

NSAIDs can reduce renal clearance of aminoglycosides, raising amikacin concentrations and the risk of nephrotoxicity and ototoxicity. This has been best described in preterm neonates but renal risk also applies to adults with reduced kidney function.

What this interaction means in plain words

Amikacin and Diclofenac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amikacin belongs to Anti Bacterial; Diclofenac belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAID inhibition of renal prostaglandins lowers glomerular filtration, reducing aminoglycoside elimination, in addition to additive nephrotoxicity.

What to do

Monitor amikacin serum levels and renal function when combined, especially in neonates, the elderly or renal impairment. Prefer an alternative analgesic where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
