Summary
Amiloride is often combined with loop diuretics to limit potassium loss and enhance diuresis. Ethacrynic acid promotes potassium excretion and the two effects counterbalance, but fluid and electrolyte disturbances, including hypovolemia, hyponatremia or hyperkalemia, may still occur.
What this interaction means in plain words
Amiloride and Ethacrynic acid have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Amiloride belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Ethacrynic acid inhibits the Na-K-2Cl cotransporter and increases potassium loss, while amiloride blocks ENaC and conserves potassium.
What to do
Monitor serum electrolytes and renal function and adjust the doses of each agent to the potassium result.
