Summary
Ethacrynic acid is a potent loop diuretic and combining it with a thiazide can cause marked diuresis, volume depletion, hypotension, hypokalemia and hyponatremia. Candesartan may add to hypotension and renal impairment in volume-depleted patients.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Ethacrynic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Sequential nephron blockade with a loop diuretic and a thiazide produces additive natriuresis and potassium loss.
What to do
Monitor electrolytes, renal function, weight and blood pressure closely, and reduce diuretic doses if volume depletion develops. Replace potassium and magnesium as needed.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
