Summary

Ethacrynic acid is a potent loop diuretic that, with hydrochlorothiazide, produces sequential nephron blockade with marked diuresis and loss of potassium, sodium and magnesium. Together with an ACE inhibitor this may cause severe hypotension and acute kidney injury, particularly in volume-depleted patients.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Ethacrynic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive natriuresis and electrolyte wasting from loop and thiazide diuretics, plus ACE inhibitor reduction of glomerular filtration pressure in volume depletion.

What to do

Monitor blood pressure, volume status, electrolytes and creatinine closely, particularly in the first days. Reduce or hold the diuretics if hypotension, hypokalemia or rising creatinine occurs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
