Summary

Both drugs interact with P-glycoprotein. Berotralstat inhibits P-gp and can increase amiodarone exposure, and amiodarone may in turn raise berotralstat levels. Berotralstat also prolongs QT at higher doses, which adds to amiodarone's own QT effect.

What this interaction means in plain words

Amiodarone and Berotralstat can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Berotralstat belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Mutual P-gp inhibition raises exposure to both drugs, with additive QT prolongation as a pharmacodynamic contribution.

What to do

Monitor ECG for QTc prolongation and watch for amiodarone toxicity such as bradycardia. Correct potassium and magnesium abnormalities.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
