Summary

Ifosfamide is activated to its active metabolites partly by CYP3A4 and CYP2B6, and amiodarone inhibits CYP3A4. This could reduce activation and efficacy while possibly increasing neurotoxic metabolite formation, but the clinical effect is not well established.

What this interaction means in plain words

Amiodarone and Ifosfamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Ifosfamide belongs to Oncology.

Evidence level: Low

Why it happens

CYP3A4 inhibition by amiodarone may alter ifosfamide bioactivation and metabolite profile.

What to do

Monitor for reduced antitumor response and for neurotoxicity during ifosfamide therapy. Consider an alternative antiarrhythmic if the oncology team is concerned.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
