Summary

Tricyclic antidepressants can potentiate the cardiovascular effects of beta-agonists such as salbutamol, causing tachycardia, palpitations or arrhythmias. Amitriptyline also has anticholinergic effects that add to ipratropium.

What this interaction means in plain words

Amitriptyline and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amitriptyline belongs to Central Nervous System; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Tricyclics block norepinephrine reuptake and have additive anticholinergic and proarrhythmic effects with inhaled beta-agonist and antimuscarinic.

What to do

Monitor heart rate, blood pressure and ECG if symptoms occur, particularly with high or frequent salbutamol doses. Watch for anticholinergic effects such as dry mouth, urinary retention and constipation.
