Summary

Chlorthalidone is a second thiazide-like diuretic added to hydrochlorothiazide, so the diuretic effect is duplicated. This increases the risk of hypokalemia, hyponatremia, hypomagnesemia, hyperuricemia, volume depletion and hypotension, while valsartan only partly offsets potassium loss.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Chlorthalidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Chlorthalidone belongs to Cardiovascular.

Evidence level: High

Why it happens

Duplicate inhibition of the distal tubule sodium-chloride cotransporter produces additive natriuresis and electrolyte loss.

What to do

Avoid using two thiazide-type diuretics together unless intentionally prescribed for resistant edema or hypertension. If both are used, monitor electrolytes and renal function closely.
