Summary
Drospirenone has anti-mineralocorticoid activity and can raise serum potassium, which adds to the potassium-raising effect of valsartan. Hydrochlorothiazide lowers potassium and may partly offset this, but hyperkalemia remains possible, especially with renal impairment.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Drospirenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Drospirenone belongs to Endocrine.
Why it happens
Additive hyperkalemic effect of aldosterone antagonism by drospirenone and angiotensin receptor blockade, partly counteracted by thiazide kaliuresis.
What to do
Measure serum potassium during the first cycle of combined use and periodically thereafter. Consider a non-drospirenone contraceptive in patients with renal impairment or a history of hyperkalemia.
