Summary
Fludrocortisone causes strong mineralocorticoid-driven potassium loss, which adds to the kaliuretic effect of hydrochlorothiazide and may cause significant hypokalemia. It also causes sodium and fluid retention that opposes the antihypertensive effect of the combination.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Fludrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Fludrocortisone belongs to Endocrine.
Why it happens
Additive renal potassium excretion from mineralocorticoid receptor activation and thiazide diuresis, with opposing sodium retention.
What to do
Monitor serum potassium and blood pressure regularly and supplement potassium if needed. Valsartan may partly offset potassium loss, but do not rely on it.
