Summary
Flurbiprofen can blunt the antihypertensive effect of valsartan and hydrochlorothiazide through sodium and fluid retention. Together with an ARB and a diuretic it increases the risk of acute kidney injury and hyperkalemia, especially in older or volume-depleted patients.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Flurbiprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Flurbiprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and promotes sodium retention, impairing glomerular filtration when angiotensin II action is blocked.
What to do
Use the lowest effective NSAID dose for the shortest period, keep the patient hydrated, and monitor blood pressure, creatinine and potassium. Prefer a non-NSAID analgesic where feasible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
