Summary

Triamterene is a potassium-sparing diuretic and combined with valsartan can cause clinically important hyperkalemia, especially with renal impairment, diabetes or advanced age. Hydrochlorothiazide lowers potassium and partly counters this, though the net result is unpredictable. Triamterene with an ARB also raises the risk of acute kidney injury.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Triamterene can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Triamterene belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive potassium retention from ENaC blockade and angiotensin receptor blockade, partly opposed by thiazide-induced potassium loss.

What to do

Monitor serum potassium and renal function soon after starting and periodically thereafter. Avoid potassium supplements and stop triamterene if hyperkalemia occurs.
