Summary
Gentamicin is nephrotoxic, and olmesartan can reduce glomerular filtration pressure, so the combination may increase the risk of acute kidney injury and electrolyte disturbances. Gentamicin may also cause renal magnesium and potassium wasting, while olmesartan may cause hyperkalemia.
What this interaction means in plain words
Amlodipine-olmesartan and Gentamicin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan belongs to Cardiovascular; Gentamicin belongs to Anti Bacterial.
Why it happens
Additive renal hemodynamic and tubular toxicity, with opposing effects on potassium handling.
What to do
Monitor serum creatinine, potassium and magnesium, and gentamicin trough levels. Consider temporarily withholding olmesartan if renal function declines or the patient becomes volume depleted.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
