Summary
Effervescent aspirin at analgesic doses can reduce the antihypertensive effect of olmesartan and hydrochlorothiazide and increase the risk of acute kidney injury, especially in elderly or volume-depleted patients. Potassium may also rise. The sodium bicarbonate load can add sodium and further oppose blood pressure control.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Aspirin / citric acid / sodium bicarbonate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Salicylate inhibits vasodilatory renal prostaglandins, causing sodium and fluid retention and reduced glomerular perfusion when combined with an ARB and diuretic.
What to do
Monitor blood pressure, renal function and potassium if used regularly, and keep patients well hydrated. Prefer paracetamol for occasional analgesia where suitable.
