Summary

Low-dose aspirin has little effect on the combination, but anti-inflammatory doses can blunt antihypertensive and diuretic effects and increase the risk of renal impairment in combination with an ARB and a diuretic. Hyperkalemia risk rises when renal function declines.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Aspirin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

High-dose aspirin inhibits renal prostaglandin synthesis, causing sodium retention and reduced glomerular perfusion.

What to do

No change is usually needed for low-dose cardioprotective aspirin. With analgesic or anti-inflammatory doses, monitor blood pressure, creatinine and potassium and maintain hydration.
