Summary

Both thiazide diuretics and SSRIs can cause hyponatremia, and together the risk is increased, particularly in older adults. Thiazide-induced hypokalemia and hypomagnesemia may also add to citalopram-related QT prolongation risk.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Citalopram can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Citalopram belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effects: thiazide-induced renal sodium loss plus SSRI-associated SIADH; electrolyte depletion can potentiate QT effects.

What to do

Check serum sodium, potassium and magnesium at baseline and after starting or changing therapy, especially in the elderly. Correct electrolyte abnormalities and consider an ECG if risk factors for QT prolongation are present.
