Summary
The combination product already contains a thiazide diuretic, so adding methyclothiazide duplicates therapy. This increases the risk of hypokalemia, hyponatremia, volume depletion and hypotension, although the ARB partly offsets potassium loss.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Duplicate thiazide diuretic activity with additive electrolyte and volume effects.
What to do
Avoid combining two thiazides and stop the duplicate. Monitor electrolytes, renal function and blood pressure if overlap occurs.
