Summary
Potassium phosphate supplements raise serum potassium, and olmesartan reduces aldosterone-mediated potassium excretion, so hyperkalemia can develop. Hydrochlorothiazide offsets this partially but unpredictably.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Potassium acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Added potassium load with reduced renal potassium excretion from angiotensin receptor blockade.
What to do
Check serum potassium and renal function before and soon after starting, then regularly. Avoid supplementation unless hypokalemia is documented.
