Summary

Non-acetylated salicylates have NSAID-like effects on renal prostaglandins and can blunt the antihypertensive effect of olmesartan and hydrochlorothiazide. In volume-depleted or elderly patients, the combination can also worsen renal function. Potassium effects are variable.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal vasodilatory prostaglandins reduces natriuresis and renal perfusion, opposing the diuretic and angiotensin receptor blocker effects.

What to do

Monitor blood pressure, renal function and serum potassium, especially in elderly or dehydrated patients. Keep the salicylate dose low and the duration short, and ensure adequate hydration.

What to ask your pharmacist
