Summary
Salsalate, a non-acetylated salicylate, can reduce the antihypertensive effect of olmesartan and hydrochlorothiazide. Together with an ARB and diuretic it may impair renal function, particularly in elderly or volume-depleted patients. It may also affect potassium balance.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of renal prostaglandin synthesis causes sodium and water retention and lowers glomerular perfusion, which opposes the antihypertensives and adds to renal risk.
What to do
Monitor blood pressure, serum creatinine and potassium, particularly in the first weeks and in at-risk patients. Maintain hydration and use the lowest effective salsalate dose.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
