Summary
Vilanterol, a long-acting beta-agonist, can lower serum potassium and prolong QTc, effects that add to the potassium loss caused by hydrochlorothiazide. The risk is greatest if the beta-agonist dose is exceeded or in patients with other risk factors for arrhythmia.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Umeclidinium bromide / vilanterol inhaled have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Beta-2 agonism shifts potassium into cells, while thiazide diuresis increases renal potassium loss, producing additive hypokalemia.
What to do
Check serum potassium and magnesium periodically, particularly in patients with cardiac disease or other hypokalemia risks. Replace potassium if levels are low.
