Summary
Xipamide is a thiazide-like diuretic, so adding it to hydrochlorothiazide duplicates the diuretic class. This increases the risk of hypotension, volume depletion, hypokalemia, hyponatremia and renal impairment, and also adds to the effect of olmesartan and amlodipine.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Xipamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Duplicate inhibition of distal tubular sodium reabsorption, with additive hypotensive effects.
What to do
Avoid combining two thiazide-type diuretics unless clearly intended. If used together, monitor electrolytes, renal function and blood pressure closely.
