Summary
NSAIDs such as ketoprofen can blunt the blood pressure lowering effect of valsartan. In volume-depleted or elderly patients, or those with renal impairment, the combination can precipitate acute kidney injury and hyperkalemia.
What this interaction means in plain words
Amlodipine-valsartan and Ketoprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-valsartan belongs to Cardiovascular; Ketoprofen belongs to Musculoskeletal.
Why it happens
NSAIDs inhibit synthesis of vasodilatory renal prostaglandins, reducing renal perfusion and sodium excretion and opposing angiotensin receptor blockade, while impairing potassium excretion.
What to do
Use the lowest effective NSAID dose for the shortest time and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, particularly in the elderly or those on diuretics.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
