Summary

Barbiturates and opioids together increase the risk of profound sedation and respiratory depression. Amobarbital also induces CYP3A4, which can lower fentanyl levels and reduce analgesia, and levels may rise after the barbiturate is stopped.

What this interaction means in plain words

Amobarbital and Fentanyl can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amobarbital belongs to Central Nervous System; Fentanyl belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive CNS depression plus CYP3A4 induction by amobarbital increasing fentanyl clearance.

What to do

Avoid if possible; otherwise use the lowest doses and monitor respiration and sedation. Adjust fentanyl dose on starting or stopping amobarbital.
