Summary
Barbiturates such as amobarbital induce hepatic enzymes and can raise the biotransformation of methoxyflurane. This produces more inorganic fluoride and other nephrotoxic metabolites, increasing the risk of renal injury, including high-output renal failure.
What this interaction means in plain words
Amobarbital and Methoxyflurane are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Barbiturate-mediated induction of hepatic CYP enzymes increases methoxyflurane metabolism to nephrotoxic fluoride-containing metabolites.
What to do
Avoid methoxyflurane in patients taking barbiturates and choose a different anesthetic or analgesic. If exposure has occurred, monitor renal function and urine output.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
