Both drugs depress the CNS, so combined use can cause excessive sedation and psychomotor impairment. Barbiturates may also induce metabolism of tricyclic antidepressants, lowering protriptyline levels and possibly reducing antidepressant effect.

What this interaction means in plain words

Amobarbital and Protriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive CNS depression plus barbiturate induction of hepatic enzymes that metabolize tricyclics.

What to do

Monitor for oversedation and reduced antidepressant response. Adjust the protriptyline dose according to clinical response and counsel the patient about driving and alcohol.
