Summary

Amoxapine is a sedating antidepressant with dopamine-blocking properties, and combining it with brexpiprazole increases sedation and the risk of orthostatic hypotension. Additive dopamine antagonism raises the risk of extrapyramidal symptoms and tardive dyskinesia, and amoxapine may also raise brexpiprazole exposure through CYP2D6 effects.

What this interaction means in plain words

Amoxapine and Brexpiprazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive CNS depression and dopamine D2 blockade, with possible modest CYP2D6 inhibition by amoxapine increasing brexpiprazole levels.

What to do

Monitor sedation, blood pressure and movement disorders, and start at low doses. Watch for increased brexpiprazole adverse effects.

What to ask your pharmacist
