Summary

Esmolol can antagonise the bronchodilating effect of arformoterol, a long-acting beta-2 agonist. At higher doses esmolol loses beta-1 selectivity, which increases the risk of bronchospasm in patients with reactive airway disease.

What this interaction means in plain words

Arformoterol and Esmolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Arformoterol belongs to Respiratory; Esmolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at beta adrenergic receptors with loss of selectivity at higher esmolol doses.

What to do

Use the lowest effective esmolol dose and monitor for bronchospasm and loss of asthma or COPD control. Consider an alternative to esmolol in patients with severe reactive airway disease.

What to ask your pharmacist
