Summary

Thioridazine causes marked dose-related QT prolongation and is itself contraindicated with other QT-prolonging drugs. Adding azithromycin raises the risk of torsades de pointes and sudden cardiac death.

What this interaction means in plain words

Azithromycin and Thioridazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Azithromycin belongs to Anti Bacterial; Thioridazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation through hERG potassium channel blockade.

What to do

Avoid the combination and choose a non-macrolide antibiotic with no QT effect. If unavoidable, use continuous ECG monitoring and keep electrolytes normal.
