Summary
Pemetrexed is cleared almost entirely by the kidneys, and parenteral bacitracin is known to cause dose-related renal tubular damage. Any decline in renal function can raise pemetrexed exposure and increase myelosuppression, mucositis and further kidney injury.
What this interaction means in plain words
Bacitracin and Pemetrexed can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Bacitracin belongs to Miscellaneous; Pemetrexed belongs to Oncology.
Why it happens
Additive nephrotoxicity, with reduced renal clearance of pemetrexed secondary to kidney injury.
What to do
Avoid systemic bacitracin in patients receiving pemetrexed and choose a non-nephrotoxic alternative. If unavoidable, monitor serum creatinine and blood counts closely.
What to ask your pharmacist
