Summary

Bedaquiline prolongs the QT interval, and formoterol can cause tachycardia, hypokalemia and QT prolongation. Together they may increase the risk of torsades de pointes, particularly with electrolyte disturbances or heart disease.

What this interaction means in plain words

Bedaquiline and Formoterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bedaquiline belongs to Anti Bacterial; Formoterol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive QT prolongation, with formoterol-induced hypokalemia potentially increasing arrhythmia risk.

What to do

Obtain baseline and periodic ECGs and keep potassium, magnesium and calcium within normal range. Review the need for formoterol or consider alternatives if QTc becomes markedly prolonged.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
