Summary

Bedaquiline prolongs the QT interval, and indapamide can also contribute to QT prolongation, mainly through hypokalemia and hypomagnesemia. Together they increase the risk of torsades de pointes.

What this interaction means in plain words

Bedaquiline and Indapamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bedaquiline belongs to Anti Bacterial; Indapamide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QTc prolongation, with diuretic-induced electrolyte depletion increasing arrhythmia susceptibility.

What to do

Obtain baseline and periodic ECGs and keep potassium, magnesium and calcium within normal range. Consider an alternative diuretic if QTc becomes excessively prolonged.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
