Summary

Belladonna alkaloids have antimuscarinic activity, so combining them with ipratropium can add to anticholinergic effects such as dry mouth, urinary retention, constipation and blurred vision. Because inhaled ipratropium is poorly absorbed, the effect is generally modest at usual doses, but opium in the suppository adds constipating and CNS depressant effects.

What this interaction means in plain words

Belladonna and opium and Ipratropium have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Additive antagonism of muscarinic receptors.

What to do

Monitor for urinary retention, constipation and dry mouth, especially in older patients or those with prostatic hypertrophy or glaucoma. Reduce the belladonna dose or stop it if anticholinergic symptoms develop.
