Summary

Combining an ACE inhibitor with an angiotensin receptor blocker gives dual renin-angiotensin system blockade. This raises the risk of hyperkalemia, hypotension and renal impairment, and the benefit is limited to selected heart failure patients under specialist care.

What this interaction means in plain words

Benazepril and Candesartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Benazepril belongs to Cardiovascular; Candesartan belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive pharmacodynamic inhibition of the renin-angiotensin-aldosterone system at two points, reducing aldosterone and glomerular efferent tone.

What to do

Avoid routine combination and use a single RAS blocker. If used in heart failure, monitor potassium, creatinine and blood pressure closely.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
