Summary

Drospirenone is a spironolactone-like progestin with antimineralocorticoid activity that can raise serum potassium. Combined with an ACE inhibitor, which also reduces potassium excretion, hyperkalemia can develop, particularly in patients with renal impairment or diabetes.

What this interaction means in plain words

Benazepril and Drospirenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Benazepril belongs to Cardiovascular; Drospirenone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive potassium retention through ACE inhibitor-mediated aldosterone suppression and drospirenone's antimineralocorticoid effect.

What to do

Check serum potassium and renal function during the first cycle of combined use and periodically afterward. Consider a different contraceptive in patients with renal impairment or other hyperkalemia risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
