Summary

Volatile anesthetics such as isoflurane cause vasodilation and myocardial depression, and patients on ACE inhibitors may develop marked hypotension during anesthesia. The hypotension can be refractory to usual vasopressors.

What this interaction means in plain words

Benazepril and Isoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Benazepril belongs to Cardiovascular; Isoflurane belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive vasodilation with blunting of angiotensin II-dependent blood pressure compensation.

What to do

Anesthesia staff should be aware of ACE inhibitor use and monitor blood pressure closely. Consider withholding benazepril on the morning of surgery per local protocol and treat hypotension with fluids and vasopressors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
