Summary

Corticosteroids given with non-depolarizing neuromuscular blockers, especially during prolonged ICU use, are associated with acute myopathy and prolonged weakness. This can delay weaning from ventilation and recovery.

What this interaction means in plain words

Betamethasone and Pancuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Betamethasone belongs to Endocrine; Pancuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Probable additive muscle injury from steroid-induced catabolism and prolonged denervation-like effect of neuromuscular blockade; not fully established.

What to do

Limit the dose and duration of both agents and monitor neuromuscular function. Check for persistent weakness and CK if myopathy is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
