Summary

Bosentan is a moderate CYP3A4 inducer and may modestly lower sorafenib exposure. Reduced sorafenib levels could weaken antitumour effect, although sorafenib is cleared by both CYP3A4 oxidation and UGT1A9 glucuronidation, so the effect is expected to be limited.

What this interaction means in plain words

Bosentan and Sorafenib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bosentan belongs to Miscellaneous; Sorafenib belongs to Oncology.

Evidence level: Low

Why it happens

Bosentan induces CYP3A4, increasing the oxidative metabolism of sorafenib.

What to do

Monitor for reduced tumour response when bosentan is started or stopped. Consider an alternative to bosentan if loss of sorafenib efficacy is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
